POLICY FOR TRANSFER / POSTING AND POSTGRADUATE TRAINING FOR MOs/WMOs (BS.17) AND SMOs/SWMOs (BS-18) – In suppression of this department’s Transfer/ Posting and Postgraduate Training policies dated

29.05.2018, 21.08.2019 and 10.05.2022, the Competent Authority has been pleased to approve the following policy for the transfer/posting and for grant of Postgraduate Training in favour of Medical Officers / Women Medical Officers (BS-17) & Senior Medical Officers / Senior Women Medical Officers (BS-18):

1. Transfer / Posting:

i. All fresh appointees of Punjab Public Service Commission will be posted in the DHQ Hospitals (Non-Teaching), THQ Hospitals, RHCs, BHUs, GRDs etc. across the province, except District Lahore.

ii. All MOs/WMOs of PPSC Batch 2012 and earlier are eligible for transfer / posting in District Lahore / disposal of SH&ME Department, without any requirement of periphery service.

iii. All MOs/WMOs of PPSC Batch 2013 and onwards are required to serve in the periphery i.e., DHQ Hospitals (Non-Teaching), THQ Hospitals, RHCs, BHUs, GRDs etc. for a period of 3 years, before they become eligible for transfer / posting in District Lahore.

iv. All MOs/WMOs of PPSC Batch 2013 and onwards are required to serve in the periphery i.e., DHQ Hospitals (Non-Teaching), THQ Hospitals, RHCs, BHUs, GRDs etc. for a period of 3 years, before they become eligible for transfer / posting to SH&ME Department.

2. Postgraduate Training:

  1. Those MOs/WMOs who have not served in the periphery for at least two years, will be considered for grant of EOL (without pay) to undergo Postgraduate Training, as per the Revised Leave Rules, 1981.
  2. Those MOs/WMOs who have completed at least two years’ periphery service will be considered for placement in teaching hospitals to undergo their Postgraduate Training, only, if the concerned training institution is ready to bear the expenditure of their salaries during deputation period.

i. They will provide affidavit that they will serve in the P&SH Department for the years / period at least equivalent to their Postgraduate Training.

ii. In this regard, Postgraduate Training shall only be allowed in the Institutions functioning under the administrative control of the Federal Government, Provincial Governments, Armed Forces Medical Colleges and also in those Autonomous Institutions that are attached with any Government Department.

iii. Those MOs/WMOs who have undergone postgraduate training and have passed Diploma Courses / MCPS / FCPS / MD I Equivalent Qualification shall serve in P&SH Department / SH&ME Department, at least for a period of 3-years before joining next training (except if they have already completed 3 years periphery service).

3. Supra-specialties:

i. All MOs/WMOs Whose specialties are not available in P&SH Department or have no sanctioned posts in P&SH Department will be transferred to SH&ME Department, subject to provision of NOC from SH&ME Department The list of specialties for which there are no sanctioned posts in PS&H Department is attached as Annex-A. and may be updated from time to time.

ii. For every induction batch who has completed theirtraining in supra-specialties, the committee consisting of the following members as mentioned below shall notify the policy parameters, every 6 months, to allow their placements, in SH&ME Department:

b. They shall not seek transfer from one SH&ME Department Institute to another against policy parameters.

c. They shall serve in P&SH Department if they do not join at the institute the Committee has approved as per policy parameters.

iv. All MOs/WMOs who are selected for 2nd fellowship in Supra-specialty, will be transferred to SH&ME Department, after serving in P&SH Department for at least one year, subject to provision of NOC from SH&ME Department.

4. Miscellaneous:

i. The period served in the Prisons / Jails, under the Home Department or any other provincial government entity by MOs / WMOs / SWMOs / SMOs, may also be considered as periphery service except in District Lahore.

ii. The requests for transfer I posting on wedlock basis will be treated as per provisions of this policy.

iii. Upon promotion of MOs/WMOs to the rank of SMO/SWMO or equivalent, they will be allowed to continue to serve at their present places of posting subject to vacancy. iv. This policy is subject to revision by the Competent Authority

WFME RECOGNITION PROCESS INITIATED BY PMC – Pakistan Medical Commission has formally initiated the application process for recognition by World Federation for Medical Education (WFME). The entire process of assessment including a physical visit by the WFME and inspection is expected to take 12 to 15 months, which is well within the time frame of 2024 when WFME recognition will be mandatory to enable Pakistani doctors and students to study or work abroad and specifically in compliance to the requirements of the authorities in the United States. The WFME recognition is essential for Pakistan to ensure that doctors from Pakistan are able to go and train and work internationally.

The PMC will be closely liaising with WFME and other international organizations to ensure that all requirements for recognition are fully met prior to the actual assessment and inspection process of WFME. In addition, PMC as part of the process planned for extensive workshops and seminars for all medical colleges in Pakistan over the next four months to assist in capacity building and enable and prepare all medical colleges for the WFME visit and inspections.

The WFME recognition process initiated runs its full course divided over phases with extensive data and information submissions made and evaluated as per the steps of the WFME process culminating in the final physical visit and assessment of the PMC and selected medical colleges.

WFME RECOGNITION PROCESS INITIATED BY PMC

It has taken the Commission extensive efforts and continued work for almost 18 months to prepare for the WFME process, which is now complete. The formal application to WFME to initiate the process required the prior approval of the new 2022 National Standards for Recognition of Medical and Dental Colleges and Teaching Hospitals as well as the 2022 National Medical and Dental Curricula, which have recently been approved and notified after the invaluable and immense effort including hundreds of hours of work put in by the National Medical & Dental Academic Board over the last 15 months to formulate and structure these standards and the curriculum. The standards and curriculum provide the foundation for modern medical and dental education systems and the Board will be continuing their work to consistently improve the standards and curriculum raising the quality bar for both colleges and students.

The Commission has over the last year worked tirelessly to ensure that all regulatory processes including the recognition and accreditation of teaching hospitals and colleges as well as the admission and examination process and especially the national licensing examination are designed and implemented at the highest international standards including the introduction of digital licensing for practitioners. These are key aspects required for the WFME recognition process. The Commission had unfortunately inherited a system that was based on outdated and manual processes without the proper transparency or objectivity required in a modern regulatory structure. The Commission’s digital transformation has been one of the key ingredients which have enabled the large-scale reforms to the medical and dental sector regulation.

The Pakistan Medical Commission is also a Full Member of the International Association of Medical Regulatory Authorities (IAMRA). PMC is today internationally recognized as the exclusive regulatory body in Pakistan following the highest standards for medical and dental education and licensure of medical and dental practitioners.

As part of its efforts to collaborate with international regulatory authorities and ensure its standards are recognized in key countries where Pakistani doctors seek to undertake postgraduate training and gain work experience, the Commission is working on finalizing a technical relationship with some of the leading international medical regulatory authorities specifically with a view to acquiring a transfer of knowledge and technical expertise in enhancing the standard of the Pakistan National Licensing Exam. The objective remains to have the NLE recognized internationally to provide further recognition to Pakistani medical and dental graduates.

I am directed to refer to the letter bearing No. FD.SR.V.3-1/2021 dated 25th July 2022 received from the section Officer (SR-V), Finance Department along with a copy of notification bearing No. FD.SR.V.3-1/2021 dated 20th July 2022 on the subject and to state that the Government of Punjab has granted Special Allowance – 2022 with effect from March 1st, 2022 and till further orders to the Civil Servants of the Punjab Government in BS-1 to BS-19 @ 15% of Basic Pay Scales, 2017 and contract employees employed against the civil post in Basic Pay Scale BS-1 to BS-19. The primary characteristic of this notification is that the basic pay scales have been changed by the Punjab Government after roughly five years.

How much will the base pay increase?

The foregoing Adhoc relief allowances are combined, increasing the employees’ Basic Pay by almost 1.5 times. Their base salary increased, but since these allowances have been combined with base pay, they will no longer get them. I’ve created a basic pay scales chart for 2022 to fix the pay from point to point in order to verify the new basic pay.

Benefits of Pay Scales Revision

The fundamental pay scales will be revised, increasing benefits for employees. The following are a few of the advantages:

1. The main advantage of this is that should the government award any basic pay allowances in the future, they will have a higher value than they did under the prior pay scales.

2. More leave encashments will occur (Nearly 1.5 times more)

3. Retiring staff will receive increased commutation and gratuities (Nearly 1.5 times more)

4. As one basic pay, they will receive an extra Transfer Grant. 

Adhoc Relief Allowance 2022 Punjab @ 15% of Running Basic Pay (2017 Scales)

The employees would additionally get an Adhoc Relief Allowance in 2022 at a rate of 15% of their current basic salary under the pay scales in 2017. With effect from July 1, 2022, employees in the Punjab province will get this allowance (ARA-2022). This ARA-2022 will be provided to all BPS-01 through BPS-02 workers.

Additionally, the Finance Department of Punjab has published a Notification on a special allowance for employees for the year 2022 at a rate of 15% of the 2017 Basic Salary Scales’ existing basic pay. This allowance will be given to BPS-01 through BPS-19 personnel who do not currently get any post-specific compensation, etc. On August 1, 2022, they will receive a one-month regular allowance as well as a four-month arrears amount. The federal government awarded the Disparity Reduction Allowance 2022 Federal @ 15%, and employees of the province of Punjab will begin receiving it on March 1, 2022. The provincial employees will receive their arrears payments for March, April, May, and June 2022 in this fashion as well.

The following benefits for employees were mentioned by the finance minister in the Punjab Budget Speech 2022, and the government has officially notified all of them:

1. A 15% pay raise as an Adhoc Relief Allowance

Clarification on Adhoc and Special Allowance 2022 – For the benefit of government employees, particularly those in the Punjab Province who received both the Adhoc and Special Allowance 2022, I am releasing the Clarification Adhoc and Special Allowance 2022 here. I have thoroughly described the entire issue in order to clear up any ambiguity:

The admissibility of Adhoc Allowance and Special Allowance is unclear to the employees. On Facebook and Gmail, I have seen a lot of comments regarding the clarification. Reality of ARA-2022 and SA-2022

Recently, the Punjab Finance Department published a notification for Special Allowance 2022 @ 15% on the Basic Pay of the Running Pay Scales of 2017. The Finance Division issued Adhoc Relief 2022 on the running basic pay of the 2017 pay scales in a similar manner to the Notification of Revised Pay Scales 2022 Punjab. Here, the essential components of basic wage are as follows:

1. The First Basic Salary of the 2017 Pay Scales

2. Current or Active Basic Pay Scales for 2017

3. The current or ongoing basic pay scales for 2022

Based on the aforementioned three basic salaries, the employee would receive these allowances. However, the second option was indicated in the notification by the finance department. The second option is explained as follows:

Here, the term “current or running basic pay” refers to the Basic Pay that the employee was receiving as of June 30, 2022. For newly hired employees, this basic pay may be their first, and for those who have worked for the company for more than a year, it may be their first salary plus annual raises, etc. The initial basic wage no longer includes these allowances. Similarly, the employees would not receive these allowances based on their running basic salary under the Revised Pay Scales 2022. Prior to the adjustment of the Basic Pay rates in 2022, they will get these allowances based only on basic pay.

Example

Consider a BPS-11 employee who will be paid 16970 as a base salary on June 30, 2022. His Special Allowance for 2022 will be 16970 x 15/100, or 2545 per month. Even if the increased basic salary in the 2022 pay scale is 25200/- and the first basic pay is 12570/-.

Fabrication in MLC | Declaration of Fabrication | Full Detail – This is an important article. I’ve seen a lot of questions from young physicians who are concerned about fabrication. This prompted me to write a follow-up piece. Declaring a wound to be fabricated is a difficult process.

Wound fabrication is a very common phenomenon in medicolegal cases. This practice has become quite prevalent even in modern-day due to various reasons such as the rise of new technologies, lack of forensic laboratories, and also because of the unavailability of the accused during the investigation. Forensic pathologists should be aware of the fabrication of wounds in order to prevent their misdiagnosis or misinterpretation.

Wound Categories

Wounds are injuries to living tissues and organs caused by physical aggression or mechanical trauma. They heal within a period of time and leave scars. However, some wounds do not heal properly after receiving medical care. These wounds are known as chronic wounds.

Chronic wounds are defined as wounds that don’t heal normally because of underlying diseases, such as diabetes and cancer. The body reacts differently to various types of wounds. Some wounds heal very slowly, whereas others heal quite rapidly.

Fabricated wounds fall into two categories, i.e., real and simulated/fake. In the case of real wounds, the cause of injury would be visible, whereas, in fake ones, injury is caused by external force applied to simulate the appearance of genuine or natural injuries. The role of forensic scientists is to correctly identify these wounds using the appropriate tools and techniques.

A wound fabricated for the purpose of murder or causing death is known as a homicide. When a fabricated wound is found on the body of a deceased person then it becomes homicide. Forensic science plays a vital role in determining whether the wound was self-inflicted or inflicted by someone else. In some instances, the wound may look similar to a self-inflicted wound but can be distinguished by several means. Therefore, it is important for the forensic pathologist to consider all possible causes and circumstances before reaching a conclusion.

Declaration of MLC – Declaring an MLC

Remember that declaring an MLC fabricated will result in it being questioned. Why is it so frequently challenged? It’s because fabrication is a criminal in and of itself. If you claim fabrication, the other party will have a very good case. So, MLC, the party you just accused of fabrication will confront you.

This implies that even if it appears to be fabrication in sections A-1, F1, or L-2, you should not declare it. The reason for this is that these sections are insignificant, and no lawsuit will be filed under them.

Higher sections, such as A2 and above or F2 and above, should be declared fake if they are. This will discourage fabrications, and no innocent people will be imprisoned as a result of your actions. It’s a huge responsibility. Be truthful to yourself. However, you should take a few measures before claiming fabrication.

If you’re going to declare a wound faked, include other departments such as surgery and radiology and acquire their professional judgments. Until the investigation and expert opinions are completed, do not declare it fabricated. Keep it KUO (kept under observation). The reason for this is that you may see a gunshot wound on your forearm but the bullet is still within. You recommend X-Ray and state that it is not fake. However, when the X-Ray is returned, the bullet is visible on the bone, and it seems like the bullet was surgically implanted there. So, you understand it’s a fabricated bullet wound after watching the X-Ray, but you’ve already certified it is not fabricated.

If you now alter your position on the fabrication from not fabricated to fabricated (which you may do), the party will accuse you of taking money and changing your mind. This will generate a lot of questions and headaches. When you submit an X-Ray of a completely basic A-1 for formality and it comes back with a displaced fracture of the skull, it’s a similar situation. Your clinical results do not match the X-Ray, indicating that either the fracture was created after the MLC or the X-Ray technician was paid to create a bogus X-Ray. So, before declaring fabrication, always wait for all investigations to be completed.

If you’ve made up your mind about fabrication, seek expert advice. This includes specialists such as surgeons, dentists, and radiologists, among others. If you have sought professional advice, then accountability is shared. It’s also becoming increasingly improbable that you were paid to declare a lie (The party will always challenge it saying you took the money). Before declaring fabrication, wait for a formal answer from experts in the field. The same reasons apply as in investigations. Until then, keep it KUO.

Fabrication of wounds in Medicolegal Cases

Wound fabrication is often a very important issue during medicolegal investigations. The problem with some wounds is that they are not authentic. What kind of evidence can be used to prove or disprove the authenticity of wounds?

Wounds happen to people due to various reasons such as accidents, violence, surgery, illness, etc. Some wounds are fake or fabricated intentionally. These wounds are often presented in court as the real ones. This type of case is called Medicolegal fraud. Medicolegal fraud usually happens in medical negligence cases. For example, the doctor might fabricate a wound to make it appear that patient received treatment when he was not at fault.

Fabricated wounds are commonly used as false evidence in criminal proceedings. Forensic science experts study such issues to discover whether a wound is genuine or fabricated. In forensic medicine, wound examination is a major component of the medicolegal investigation. Doctors can examine the wound using a number of methods such as X-Ray, CT scan, MRI, ultrasound, etc. Such examinations can reveal certain characteristics of the wound that can help doctors determine its origin or authenticity.

Point to Ponder!

When it comes to medicolegal cases, do not listen to any senior, dispenser, DC, CEO, or Prime Minister of Pakistan. Because you are the only one who can conduct an investigation. Others will not seek your assistance. You’ll have to fend for yourself. In MLC, no one can force you to do anything. They are meaningless threats. The ramifications of declaring a fraudulent MLC are significantly harsher than anything the CEO or any other authority can do to you. So do what you think is correct.

You should be able to justify your Fabrication. When writing a description of the wound, including facts about why you believe it is fake. If the patient claims he was struck by a brick, but the wound is incised (from a sharp weapon such as a knife or blade), then specify in the description that the weapon reported by the patient and the wound do not match. This will assist you to recall why you said it was fabricated in the future. In addition, the board assessing your MLC in question will be aware right away that you saw the patient in detail. If your MLC is self-explanatory, they usually do not ask you to come in for an inquiry.

Drink plenty of water every day

1. Food should not contain any additives that are harmful. If your food contains sugar alcohols, artificial sweeteners, preservatives, colorings, flavor enhancers, etc. then they are considered “unhealthy”. Vitamins and minerals help us get strong bones and are necessary for the proper functioning of our immune system. Antioxidants protect against cancers and other chronic conditions.

 4. Be aware of added sugars. Sugar-sweetened beverages contribute to obesity and tooth decay. When choosing juices, sodas, energy drinks, smoothies, sports drinks, fruit drinks, and sweet teas look for low or no-sugar options. Aspartame has been associated with headaches, seizures, depression, fatigue, and even death. Artificial sweeteners are often linked to weight gain, gastrointestinal disorders, and hyperactivity in children, among others.

 5. Limit salt. Salt can increase urination, decrease appetite, and raise blood pressure. Some people are sensitive to sodium. Sodium can make people feel thirsty and causes them to drink more liquid. Too much sodium can also result in dehydration and an increased risk of osteoporosis.

 6. Eat whole grains. Whole grains include bran, germ, and endosperm. The germ contains vitamin C, riboflavin, thiamine, niacin, folate, iron, magnesium, zinc, B12, biotin, and lignans. Whole grain products are higher in fiber content than refined grains. Eating too many processed carbohydrates contributes to insulin resistance, fat storage, and inflammation in the body. Types of Insomnia

There are different types of insomnia, which can be broadly classified according to how the person is trying to fall asleep.

  2. Types of Sleep Disorders There are three classes of symptoms that indicate someone has a sleep problem and each class typically covers a variety of different symptoms:

  1. Major sleep disorders: insomnia, obstructive sleep apnoea (OSA), narcolepsy, and restless legs syndrome
  2. Significant restlessness or circadian rhythm problems such as shift work disorder at night; periodic limb movement disorder during the day
  3. Light snoring with more severe daytime arousals than those experienced by people who don’t have trouble falling asleep.
