So, when you’re consuming fast food, the meat isn’t really meat, and even if it is real meat, it’s not meat the way it’s supposed to be.

Some of the sodas that you get at fast food restaurants have 8 to 12 teaspoons of sugar, and it’s not even sugar, it’s corn syrup, which is worse than sugar. High-fructose corn syrup, terrible for the liver, etc. And now there are diet sweeteners, which we believe are even worse than that. Pink, yellow, blue packets, i believe can cause brain issues. i believe they can cause gut issues, immune issues. There’s really no good way to consume some of these sweetened beverages. When you get into the desserts at fast food restaurants, they not only contain sweeteners, they contain chemicals, the ice cream etc. I see all these commercials have these smoothies that are all these crazy colors that you know don’t occur in food. I bet some of those smoothies have 40, 50, 60 grams of sugar per serving. Ask your fast food restaurant and see. It’s pretty scary. That is a cocktail that can absolutely lead you down a road you do not want to go.

We know that a lot of these sugars today contribute to cancer, obesity, diabetes, insulin issues which can affect every hormone in your body. Migraine-trigger sauces, you know, many of these products today, crazy enough, still contain things like MSG, monosodium glutamate and I mean, that’s just one. There are so many other things that they’ve added, and created, and put in products today, you know, many of the gluten-containing compounds people have triggers to. So, the big thing here is, make sure it’s real food. When you’re looking at your sauces, flip over the bottle. It should have a shorter ingredient list. You should be able to read and understand what’s in the container and what’s in the food you eat.

 

Number Seven, inflammatory oils. You know, every fast food restaurant you go to, for the most part, is using vegetable oils which are high in polyunsaturated fats, which are very sensitive to heat. When you heat these oils, it really creates oxidation, which, we know this, when you take a bite out of an apple, it starts turning brown, that’s oxidation. When somebody, as they age, starts to get brown spots on their skin, that’s, in a way it’s, you know, oxidation internally in the body. Inflammatory oils are one of the worst, along-with processed sugar, for causing inflammation and oxidation in your body, which is related to, pretty much, every disease out there, especially heart disease, diabetes, and cancer. Stay away from these inflammatory oils and switch to healthier oils, like coconut oil, ghee, and avocado oil.

 

When you subject your children or yourself to eating fast food, I would say that the biggest issues, there are three, there are probably 20, but blood sugar leading to metabolic syndrome, fatty liver, and diabetes, I’ve seen it happen, number one. Number two, brain issues. So, when you’re dealing with all these chemicals ,blood sugar going up and down, you’ve got serotonin issues, you’ve amyloid plaque issues, which means depression, Parkinson’s, Alzheimer’s, all of that, right, and then gut issues. If you eat a fast food meal, there may be 100 ingredients in there, and I wonder how many of them are food. But, this is a big one. There are a lot of people out there that are very concerned about our environment, and I will tell you this. Fast food restaurants and a lot of our industrial food supply is destroying the environment. You and I have been talking about this for a month, really delving into how to regenerate our planet. But, the environmental destruction of monocrop agriculture that’s required for all the breads, grains, cookies, pastas at fast food restaurants, pizza dough, etc., it’s rampant. Then, you add the meat, and the animal foods on grains, corn, soy, etc. So, all of that leads to that barren erosion you’re seeing on the screen because we are destroying our forests, we are forced to grow crops that require added chemicals, particularly petrochemicals where we’re using our oil supplies up, and ultimately, it is leading to issues. The sprays have GMO. We didn’t even mention the glyphosate exposure, the pesticide exposure, from consuming the breads, etc. This is a cocktail for disaster. At the end of the day, our population’s growing, more people are eating fast food, and the Earth’s resources are being destroyed. If you don’t care enough about your health to stop putting money in the pockets of fast food restaurants, care enough about our planet. At the end of the day, we need to be producers, not just consumers. And I didn’t know this. I was just like anybody else

 

But, I came to the realization that we absolutely have to leave the world in a better place, and it’s not just cliché. and the truth is, eating fast food, actually is contributing to the destruction of the planet, where eating real food, especially organic foods, whether it be an apple, or brown rice, or whatever it is, can actually help in healing the planet. So, again, stay away from the things I wrote down in this article. These are, for many people, you may not realize, or didn’t realize, all of the side effects of eating fast food. Three Things to Love your work & Professional Fulfillment

From last few weeks, I realized that, for me, my personal professional fulfillment comes down to three things, autonomy, expectations and people. So, what do I mean by that?

 

Well, first is AUTONOMY; if you feel like your work actually matters, you’re going to be a lot happier than if you feel like your work just does no one any good. So, for example last Friday in the operating room, I had a lot of autonomy. We were doing some simpler cases, therefore, as the new resident, the attending and the other residents were comfortable letting me take charge and do most – almost all of the case actually. And that’s pretty huge, I wasn’t expecting that, and that was awesome, I really enjoyed it.

 

The next point was EXPECTATIONS; If you go into a day expecting that you’re going to get out at, you know, 6:00 AM and you’re there until 8:00 PM or 9:00 PM, you’re going to be really annoyed after 6:00 PM. So, don’t do that. Expect for the worst and when things aren’t as bad as you were expecting, you’re just going to be a lot happier. Likewise, if you expect yourself to have a lot of autonomy in a case or for a case to go a certain way and it goes south or you know, you mess up, someone gets mad at you or whatever it is, if you’re so caught up in your expectations for good or for things to be great, then when the going gets tough, you won’t be able to adapt and you won’t be happy.

 

Three Things to Love your work & Professional Fulfillment

And the last and final point comes down to PEOPLE, right? So, there’s a place to anyone in every profession. When people are choosing specialties in medicine or medical college, often times what you gravity towards are those specialties that have like-minded individuals where you feel like you get along, you click well et cetera. Now, there’s nothing wrong with that but you have to be careful because people at one institution for example, like a medical college, may have a certain culture being a certain way, but when you go to another institution, they may be completely different. So, you have to be careful to acknowledge and understand the influence of people on how you perceive your work. Think back to your classes, if you have – if you are with your best friends and even the – the most mundane class, it instantly became fun, right? So, the same thing applies here. Try to find people that you do enjoy working with. So, when I was ranking my residency positions, how I got along with residents and attendant’s was very important to me because I was going to be working with these people for the next six years. If you do find a group of people that you click with, then absolutely by all means, you know, maximize it, and try to join them, enjoy yourself, and push each other to improve.

3 Tips to stay Motivated, Positive & Productive

Hey Guys, recently, I’ve been receiving this question a lot and it’s, “How do I stay productive, motivated, on the grind et cetera?

 

First of all, let me say that I am human, we all have good days and we all have bad days. I don’t wake up every single morning ready to work. Now, before I go to the three things that help me the most to stay motivated, let me first say that work-life balance is key. I don’t believe in just grinding it out day in and day out all the time. I believe in spending your time wisely. So, work, grind it out and then play, enjoy yourself, relax, and get recharged. What I think is a huge waste of time is when people try to do both at once. So, you know, like my roommates back in college would watch TV while working on an assignment for school. And you’re not really getting that much work done because you’re distracted by the TV, you’re not in flow, you can’t concentrate and at the same time, you’re not really relaxing because while you’re watching TV, you’re working. So, in my opinion, I think it’s a waste of time. If you guys disagree with me, I’d love to hear your comments down below.

 

Now, on to those three things that helped to keep me motivated.

 

Number one is; I LOOK TO PEOPLE WHO INSPIRE ME OR PEOPLE THAT I ADMIRE. One of those people would of course be Elon Musk. That guy is probably a Martian and he’s trying to get back home. Sometimes we have to ask ourselves is he even human, right? So, when I’m doing something that is particularly challenging or people are telling me I can’t do it, it’s impossible, I say, “Hey, Elon Musk was able to start this company, that company, you know, he is saving the world in this way and that way. If he did all that, I’m pretty sure I can do this one tiny little thing in comparison. That’s one way that I kind of re-frame my own challenges and my own obstacles and it helps me to get inspired and to get motivated.

 

So, I talked about this biomedical incubator, and this is an example where my friends were a little skeptical, some of them didn’t want to help because they said, “Well, I don’t know if it’s really going to work.” And, I approached many different faculties. A lot were supportive but there were three, I remember that just straight-out told me, “This can’t be done, we’ve tried before, don’t waste your time, don’t waste our time.” And, that sucked to hear but at the same time, this is what I wanted to do. I wasn’t going to take ‘no’ for an answer. I was at least going to try and I said, “Hey, if Elon Musk can do all these things, I can start a biomedical incubator.” And fast forward one year, it’s well-established, we got funding, it’s successful and it’s constantly growing. So, look to people who inspire you and use that as motivation in overcoming your own challenges.

 

3 Tips to stay Motivated, Positive & Productive

Number two, and I’m sure a lot of you guys will agree with me on this one is MUSIC.If I am having a slow morning, if I’m not pumped and energized, I will just turn on, you know, maybe one of my gym playlists or one of these songs that always gets me amped and that will help shift my energy to something more enthusiastic, pumped, you know, ready to go, ready to tackle the day. And I think that starting the day off on the right foot is so important. So, most mornings I usually do start with either a podcast or some music that puts me in a good mood.

 

And number three, this is possibly the most powerful motivator for myself, and that’s REMEMBERING THAT I’M GOING TO DIE. We’re all going to die one day and I think that the looming presence of death is a great motivator in life. And I don’t mean to sound really dark when I say this, but we all are going to die and just remembering that helps me think that if I’m not spending my time deliberately every day, if I’m not, you know, effective and efficient with my work and then effective with my relaxation and being present and spending time with my loved ones or doing things that I enjoy, I’m not getting that time back.

A Fire arm is a specialized device designed to propel a projectile (shot/bullet/missile) by the expensive force of gasses generated as a result of combustion of the propellant (powder) at its base in a closed space. This combustion results in building up of optimum pressure which forces the missile out of the muzzle (mouth of a firearm) with sufficient velocity, resulting in the form of injury.

Projectile: This means an object propelled by the force of rapidly burning gases;

Other means:

Those fired from rifled weapons, such as rifle, revolver or pistol or bullets.

Those fired from smooth bored weapons like shot guns are lead (or recently steel) shots and pallets.

Other projectiles within medico-legal work are pallets and slugs in air guns, and metal scrap, nails, stones etc; in muzzle loading guns.

A bullet is made of lead which has higher sectional density (mass/diameter). So essential for a steady flight. Since Lead is a soft material and easily deformable, the bullet is jacketed either fully or partially with cuprnickle, copper or other harder casing. The tip of the bullet is known as nose. The bullet is cylindrical from the base onwards and conical towards the tip (nose).

*Wounding effects: that are the result of strike by the projectile (bullet or short-charged), flame, hot gases, wad and any other component of ammunition like broken pieces of bullet case.

*Non Wounding effects: which are caused by smoke, un-burnt powder and grease.

Firearm wound complex has four parts.

 

  1. ·         An entry wound.
  2. ·         A track with its direction.
  3. ·         Place of resting of bullet or shot charge.
  4. ·         Exit wound.

Entry wound are classified in relation to the distance of the muzzle of a firearm from the body. When a weapon is discharged, the projectile leaves the muzzle at its maximum velocity and is accompanied by a flame, hot gases under considerable pressure, soot, discharge of burnt & un-burnt particles and metallic particles. At close ranges, all these will cause injury to the body giving entry wound many special characteristics which the exit wound will lack. The appearance of the entry wound and its extent are also influenced by the type of weapon and its caliber, the nature of the projectile and powder used, the site of body hit, and the striking velocity as a result loss or velocity Of the projectile before hitting the body.

This is free from sign of burning, blackening or tattooing, and usual collars of entrance wound. It will show an atypical abrasion when the skin is supported at the exit by a belt, tight clothing, or wall (shored exit wound). A typical abrasion is irregular at the edges, lop sided, and too large as compared to the abrasion collar of an entry wound. The fibers of the clothes may or may not be turned out. An uncomplicated exit wound is usually split from within outward.

 

In 2020 there were 50,000 deaths in the USA because of firearms. 60% of them were suicides, and 37% were homicides. These figures do not include non-fatal injuries that usually leave permanent physical injuries or post-traumatic stress. All these involve delicate medicolegal problems.


Determination of Cause

Unfortunately, recording in the frenetic emergency rooms is not giving enough information to get a handle on changing these figures. Medicolegal aspects of these wounds are important in determining the cause of the wounds. Postmortem details are sometimes difficult to determine and must be on the death certificate for the family, insurance, and legal or police work.

Today it is important to determine whether the wounds were self-inflicted on purpose or accidentally if the injuries might be homicide or they were accidental.

Modern medicolegal systems use tools and equipment to analyze, finding results more inexpensively and with more accuracy. The postmortem computer tomography is used with firearm wounds to follow bullet trails and to recognize bullet fragments. This powerful machine helps to determine what happened in the case of ‘tandem bullets’, that is two bullets released with one pull of the trigger. Because the bullets often go in different directions, this can be confusing to the trained eye.

 

 


The Problem of Interpreting

There are many reasons why a firearm wound might be misinterpreted postmortem:

  • The firearm may have been badly put together,
  • The bullet might not be right for the firearm used,
  • Another target might interfere and get in the way,
  • Or the bullet might ricochet.
  • A silencer may have been misaligned
  • The bullet might be deformed

Any of these might result in a strange wound that medicolegal personnel can’t interpret. That is when the Postmortem Computed Tomography machine can be handy at helping interpret the cases.

RELATIONSHIP BETWEEN ENTRY AND EXIT WOUND:

The Wound edges may be inverted at the entry and everted at the exit. The abrasion ring will help to distinguish entry from exit. In case of difficulty, microscopic examination and X-Ray examination of the skin or clothing may be undertaken. Evidence of carbonization swelling, homogenization of dermal collagen, presence of lead ring if any, and carboxyhemoglobin at the entry wound will distinguish it from the exit wound.

IMPORTANT POINTS REGARDING MLC

 (all credits goes to Dr Aneel Tariq for these important Points)
 
 

Patient is mostly serious if bullet hits chest or abdomen / head / neck; you don’t have time to wait and keep the victim; In that case:

 

   1.  Vitally stable the victim, don’t stitch or remove the bullets, only apply pressure bandage.

   2.  Write 2 marks of identification by yourself, you should see with your eyes (it’s important).

   3.  Color of clothes and note down number / size of holes u see at entry and exit sites. Roughly write at some paper what you think bullets correspond to respective holes in the clothing’s. Note any soot / blacking / burning at cloth fibers at site of entry of bullet. You write it all as your record to further copy it on MLC page.

    4. Now examine the wounds,

  • o   Make a drawing on rough paper, this drawing you will draw on the MLC paper later on. 1stly you make sure it’s a bullet or not /superficial / deep / single entry wound, blacking / sooting / hair burn.
  • o   Is there exit? Make trajectory of path and if you don’t see any exit write no exit but there can be swelling redness on opposite side which you will mention.

5. If there are multiple bullets fired, see through-through, does two superficial entry-exit point has a connection? Do it with a pencil or scissor but do not open any closed wound, also note and scratch the clotted blood at wounds, sometimes they are just scratches and no bullets with no depths. Don’t listen to attendants they will force you to believe it’s a firearm.

6.  You keep the docket of police in MLC register which you will fill after patient is referred to tertiary care hospital, or some other government hospital in another city but On your emergency slip you will write MLC NUMBER / identity marks/ clothing’s which will be saved in that hospital.

    7.  After all of this, you refer victim and advised x ray / CT / ultrasound on slip.

   8.  Make 2 separate slips; write please give expert surgical and radiological opinion with procedural / operational Notes or findings.

 

You send the patient, keep an attendant with you to fill in the MLC register, you write all you wrote on rough paper.

 

You hand over the MLC / docket to police. Your work is done.

 

9.  For rare cases where you suspect it’s fabricated or not a firearm wound or where you want to linger on bad attendants….. Foran insaaf ki farahmi na krain, take clothing of patient and scrap the area / swabs or take pieces of shrapnel, seal them and send them to firearm and ballistics PFSA Lahore for kind of weapon and does the Gun powder matches on cloth and scraps. It will take a month for report to come back to you.

 

 

#Result #  after some days patient will come back for result. They will force you to declare it as firearm without tertiary care hospital notes, don’t do it ,if you declare it ,opponent party will challenge it as you will not have any proof of what happened in tertiary care hospital. Have an emergency slip and ask the tertiary hospitals 1=radiologist 2= surgery for operational / surgical / procedural notes.

 

And do write ATTESTED COPIES of everything, MS / AMS / Professor / AP can attest the procedural files.

Without it they are useless in court and you cannot declare result unless you have them as proof. Radiological report of fracture / bones / ribs etc. Surgical notes / visceral damage or not / depth of wounds.

*You will write USG/x ray/CT/ surgical notes for viscera / intubation / haemothorax in detail on MLC paper. After all this, you will write from all investigations and surgical and radiological opinions [it is likely a firearm injury]. There are special sections of firearm that only police puts.

 

Sometimes fake firearm cases come, people forces you to declare small cuts / abrasions / minor wounds as firearm. Your job is to access the clothing / wounds / history and then declare it as firearm or not. If you think it’s not firearm, declare it as blunt or sharp weapon.

 

Remember, blackening, soot, burning may not be present ,entry and exit wounds can be mixed as ‘Fankaar Qoum’ so that doctor can be dodged When you don’t see signs of close contact you write distance may be more than 3 feet, in this way self inflicted fires get away. So you write its firearm injury and subject it to I.O (investigating officer) to further investigate.

Its Better not to be TOO WEALTHY

It’s actually better to not be too wealthy.

It’s better to be middle class, and if that sounds crazy to you, let me explain. So three, different levels, right? You got poverty, I don’t think that’s a good situation to be in, if you’re born into poverty, obviously, there are a lot of obstacles in your way, it doesn’t offer nearly as many opportunities and there’s a lot more disadvantages, if you’re born or live in poverty. If you’re in the middle-class, I would argue that that is the best place to be and if you are born into a lot of wealth, there are actually a lot of problems there that you don’t face in the middle class, and I think the upside doesn’t actually outweigh those downsides. Now, I’m sure that you have some friends who are wealthy, or you know some people in school, or in your class, or some colleagues, or people on social media or whatever, you know, some wealthy people, and initially on the surface level, they may seem very happy, they may seem to have it all, oh my god, you know, his dad bought him a new Porsche and she got a new, you know, whatever, the thing is, when you actually talk to a lot of people that come from wealthy backgrounds, on average, they’re not any happier, in fact, they have bigger problems usually, or at least very different problems than what you and I would face.

 

So there’s two common situations, first, you got the trust fund babies, and this is where, essentially, the person doesn’t need to work for the rest of their life, their family, there’s this family wealth that they’re going to inherit and they’re taken care of, they don’t need to work at all, and I know some people like this. It sounds great initially, but there are some problems. So first of all, you’re gonna face that existential crisis much sooner than anyone else, and dealing with that, when you’re pretty young, is challenging. This is why you see a lot of people who are born into wealthy families, getting involved in like drugs and other not-so-good things, because it’s harder to find meaning and purpose in their life. So someone like me, I got to find a job, I got to work, and medicine was my calling, and I mean, we respond to incentives as humans, so if I was a trust fund baby, would I still have the drive and the push to go through like, to work really hard as a premed, and then, work really hard in med school and then matched into plastic surgery and do all that? I don’t know, I mean, if I knew that I could just like, coast and chill and still be taken care of, maybe I wouldn’t have the same level of drive. So I would argue that if you are born into a lot of wealth, it’s harder to be as ambitious, as driven and to find purpose and meaning in your work, you’re kind of forced to find that elsewhere, and that’s more challenging.

 

Its Better not to be TOO WEALTHY

Now, the second situation is a family business. So let’s say your mom or dad started this wildly successful family business and now, as you’re getting older, you have the choice. Do you take over the family business or do you not? If you take over the family business, that’s what you’re supposed to do, right? Your family is happy that you’re going to make good money, your family is going to be taken care of, your kids, et cetera and you don’t really have a choice, because if you say no to the family business, then, you’re going to be disowned, your parents aren’t going to be happy with you, they’re going to be pretty upset and chances are, you probably won’t make as much money, you won’t be as comfortable. So what a lot of people do in that situation, is they feel forced to take over the family business. Oh, boo-hoo, you had to inherit your dad’s 2 million per year business or whatever it is, it sounds great, but what if you really wanted to be an artist? Or you wanted to be an actor? Or you want to be a doctor even? Those options become much less appealing when you have the proposition of a business that maybe you don’t enjoy, but it’s a lot safer, it’s going to pay better, etcetera, et cetera, et cetera.

 

So if you are born middle class, I would argue you have this opportunity to light that fire under your own butt and make something meaningful. And here’s the thing, people talk about money and happiness and if there’s one thing that annoys me, it’s when people talk about that one study, the $75,000 per year income is the plateau for happiness. Turns out, net worth is a better measure than income, obviously. And there were two interesting findings that came out of a recent net worth study, and the main one I want to highlight, is that being self-made resulted in higher average levels of life satisfaction and happiness compared to a windfall. So if you win the lottery or inherit a lot of money, you’re going to be happier if you earn that and that kind of makes sense when you think about it.

 

The reason I’m writing this article, is because it’s so easy for us to get caught up in these victim mentalities, these victim mindsets, where we see how life is unfair, where we see how, you know, this other person has it easier and why is my life so hard? And this sucks. But when we actually stop and think about it, it becomes really clear how lucky we are. Even something as simple, as not being born into massive wealth, I think that actually an advantage, at least that’s the story I tell myself, it helps me sleep at night and it makes me happier, but when we stop to actually think about how awesome our lives are, so I just finished a 48-hour fast, and I ate that first bite of food and it was so satisfying and I’m so grateful to be able to eat the foods I can eat, especially after Crohn’s disease when I had to be so restrictive, and at times, I still need to be restrictive when I’m flaring, just being able to eat good food, oh my god, it’s such a joy, sucha pleasure, you know? Not being born into massive wealth, that’s actually an advantage, I feel so much more driven and like motivated and like, I’m going to do something big with my life and I don’t know if I would have that if I was born into wealth. And here’s the cool thing, whether or not you believe my arguments, that doesn’t really matter, at the end of the day you get to decide your own narrative, your own story, your own beliefs, but you alone decide whether or not those beliefs serve you or not, and obviously, this belief, that I am better off by being born middle class and working my ass off and working six months in the library in 11th grade so I could buy my first laptop, I think that story serves me and at the end of the day, only you will decide what story you take. So you can choose to be a victim and say, you know what? Life’s unfair. Or you can take ownership. What’s it gonna be?
